I understand that the massage, Fascial Stretch Therapy, or other manual therapies I receive are provided primarily for the purposes of relaxation, stress reduction, and relief of muscular tension. I acknowledge that these treatments are not intended as a substitute for medical examination, diagnosis, or treatment. I agree that I should consult a physician, chiropractor, or another qualified medical specialist for any mental or physical ailments of which I am aware.

As certain medical conditions may contraindicate massage or other modalities, I affirm that I have disclosed all known medical conditions and answered all questions truthfully. I agree to inform the therapist of any changes in my medical profile and understand that the therapist bears no liability if I fail to do so.
